Credentials
:
Bachelor’s Degree in an approved Human Services field. One year of experience providing TCM services to children/youth with SED and their families.

Special Skills/Knowledge Required
:
Willingness and ability to complete HFW Facilitator training. Must maintain a valid driver’s license with acceptable driving record throughout employment. Must have an extensive knowledge of community resources.

Primary Functions
:

• Responsible for completing a comprehensive strengths-based assessment of the individual and family.
• Ensures transportation needs are met as necessary to meet treatment needs.
• Complete service documentation and billing entries.
• Participate in skill enhancement activities, both as participant and provider.
• Obtain required continuing education hours and other training as necessary to meet state, agency and credentialing requirements.
