PositionSummary
The Activities & Nutrition Lead is a dual‑role position responsible for planning, coordinating, and overseeing both life‑enrichment programming and nutrition services within the Golden Years ADHC center in alignment with Kentucky adult day health regulations and Medicaid/HCB Waiver standards. This position serves as the designated staff member primarily responsible for dietary and activity services, ensuring participants receive safe, meaningful, person‑centered activities and meals that support their health, dignity, and ability to remain in the community.
Key Responsibilities – Activities / Life Enrichment- Plan, implement, and evaluate a structured calendar of daily activities (social, recreational, cognitive, physical, and creative) that support participant abilities, preferences, and care goals.
- Adapt activities for participants with varying cognitive and physical abilities, ensuring safe and meaningful engagement.
- Lead group sessions and provide 1:1 engagement for participants who benefit from more individualized support, focusing on reducing isolation and promoting emotional wellbeing.
- Coordinate special events, seasonal programs, and community engagement opportunities that support HCB Waiver goals of community‑based services and inclusion.
- Collaborate with case managers and nursing to align activities with person‑centered service plans and any therapy/behavioral recommendations.
- Guide and support direct care staff and volunteers in activity delivery, participant engagement, and safety practices.
- Coordinate daily meal and snack services in accordance with Kentucky adult day health regulations and applicable nutrition standards for congregate settings.
- Partner with contracted or consulting nutrition professionals (as applicable) in menu planning that incorporates participants’ medical, cultural, and personal dietary needs.
- Ensure menus and acceptable substitutions are followed as planned, with attention to portioning, texture modifications, and special diets as ordered by nursing or a nutrition professional.
- Oversee safe food handling and service practices, reinforcing procedures consistent with recognized food safety standards such as those reflected in nutrition program guidance.
- Communicate clearly with kitchen/food service staff on timing, presentation, and participants‑specific needs (allergies, swallowing risks, diabetic needs, etc.).
- Maintain required records related to menus and food service as directed by internal policy and regulation.
- Operate activity and nutrition services in compliance with Kentucky adult day health licensure regulations and HCB Waiver expectations for adult day health services.
- Maintain documentation for activities (calendars, attendance, adaptations) and nutrition (menus as served, special diet instructions) in line with policy and survey readiness.
- Participate in internal quality improvement, including preparation for surveys, audits, and waiver reviews that focus on programming and nutrition.
- Identify and report incidents, adverse events, or safety concerns quickly, assisting with follow‑up and preventive actions as needed.
