This is the entry level professional position in community based psychosocial rehabilitation services for individuals with serious and persistent mental illness. The Case Manager plans, coordinates, provides and monitors therapeutic, rehabilitative, support and community integration services for adults with severe psychiatric disability designed to assist these individuals to integrate as independently and fully into the life of the community as possible.
Case Manager works independently and must demonstrate good clinical judgment, decision making, time management and appropriate treatment for consumers in the community.
