Our PACE program helps older adults remain living safely and independently in their homes and communities while receiving comprehensive, coordinated healthcare services.
Position SummaryThe Certified
Nursing
Assistant (CNA) provides direct personal care and supportive services to PACE participants in the center and home setting. Under the supervision of the Clinical Nursing Coordinator, the CNA assists participants with activities of daily living, promotes safety and independence, observes changes in participant condition, and supports the interdisciplinary care team in delivering exceptional participant-centered care.
Assist participants with bathing, grooming, dressing, oral care, and personal hygiene.
Help participants with mobility, transfers, ambulation, and positioning.
Assist with toileting needs, including use of commodes, bedpans, and urinals.
Support participants during meals and assist with prescribed therapeutic diets.
Assist with self-administered medications as directed.
Perform delegated care tasks such as:
Obtaining vital signs
Non-sterile dressing changes
Colostomy care
Non-sterile bowel and bladder hygiene care.
Lead and participate in recreational, social, and therapeutic group activities.
Build positive relationships with participants and families.
Promote participant dignity, independence, and quality of life.
Monitor participant health and well-being.
Report physical, emotional, or cognitive changes to nursing staff promptly.
Document care provided according to organizational standards.
Collaborate with interdisciplinary team members to support individualized care plans.
Provide light homemaker and household support services as assigned.
Assist the Dietitian with meal service at the center.
Maintain compliance with all organizational policies, OSHA, CDC, state, and federal regulations.
Protect participant privacy and maintain confidentiality at all times.
Participate in required training, educational opportunities, and staff meetings.
Friday, Saturday, Sunday (3x10hr OR 3x12hr)
Saturday, Sunday, Monday (3x10h OR 3x12hr)
High school diploma or equivalent.
Current, unrestricted Certified
Nursing
Assistant (CNA) certification.Active CPR certification.
Valid driver's license, reliable transportation, and proof of insurance.
Medical clearance and required immunizations prior to participant contact.
Minimum of one
year
experience working with frail, elderly, or geriatric populations.
Experience working in home care, senior care, long-term care, PACE, or community-based healthcare settings.
Strong communication and customer service skills.
Passion for serving older adults.
Ability to lift and carry up to 50
pounds
.Ability to push or pull equipment and participants using proper body mechanics.
Frequent standing, walking, bending, and assisting participants with mobility.
Ability to work effectively in a fast-paced healthcare environment.
Ability to communicate clearly with participants, families, and team members.
