** collaborative, supportive team that values both clinical expertise and the people providing the care.**  ## What You'll DoAs a Home Health OT, you'll work one-on-one with patients in their own environment, helping them build the skills and confidence needed to live as safely and independently as possible.
* Assess patients'
** functional abilities, limitations, and home environment
*** Develop individualized treatment plans focused on independence and meaningful daily activities
* Provide skilled OT interventions to improve
** mobility, strength, coordination, safety, and activities of daily living
*** Evaluate home safety and recommend adaptive equipment or modifications when appropriate
* Educate patients, families, and caregivers on techniques that support independence and safety
* Collaborate with nurses, therapists, providers, and other members of the interdisciplinary care team
* Monitor patient progress and adjust treatment as needs change
* Complete accurate and timely clinical documentation  ## What You'll Bring
* Current
** Washington State Occupational Therapist license**, or eligible for licensure before your first day
* ** 1–2 years of Occupational Therapy experience
*** Current CPR certification
* Ability to work independently while collaborating with an interdisciplinary team
