• Provide 1:1 care for clients following their plan of care at all times while maintaining dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as needed per client's plan of care and assessments.
• Accurately document nursing actions and communicate with family, case manager, and physicians, documenting progress and outcomes for established goals and informing the team of changes in client’s medical condition.
• Coordinate or order supplies, medications, and feedings as needed to insure adequate amounts are always on hand.
• Ensure all patient needs are met in accordance with physician instructions.

• LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• CPR professional certification.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Adoration Home Health / Bright Spring Health Services is an affiliated company that recently acquired certain Amedisys home health and hospice operations. The successful candidate will initially be employed by Amedisys until January 1, 2026 at the latest. During this period, Amedisys will perform all administrative onboarding activities and offer benefits coverage under Amedisys employee benefit plans. On or before January 1, 2026, the successful candidate’s employment will be transferred to Adoration Home Health / Bright Spring Health Services.
