• Provide primary and preventative care for long‑term care patients residing in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Engage in advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with Kentucky law.
• Order, review, and follow up with treatments and diagnostic testing.
• Communicate with nursing home staff regularly to discuss changes in patient conditions and care plans.
• Collaborate with the primary care physician.

• Current, unrestricted NP license in the state of Kentucky.
• Board certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long‑term care setting.
• Proficiency with an electronic medical record (EMR).
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Collaboration skills within an interdisciplinary care team to coordinate patient care.
