Position SummaryThe Hospice RN Case Managerdeliverscompassionate and comprehensive health care services to Hospice patients in a home care setting,orwhereverthe patient calls home.
Home care settingsmayconsist ofa private residence, skilled nursing facility, personal care home, and/or other interim livingarrangements.
The Hospice RN Case Manager’sgoal for excellent patient care standardsbegins with active listening to create and implement a holistic plan of care based on the patient’s goals and preferences.
The Hospice RN Case Manager serves as the patient advocate to support, guide, and coordinate care for patients, families, and caregivers as they navigate Hospice Care.
The plan of care will include managing symptoms throughpharmacologic and non-pharmacologic interventions foroptimalcomfort, functionality, and safety. Frequent monitoring and follow-up arerequiredto evaluate the efficacy of treatments, plans of care, and to offer emotional support to both the patient and the caregiver. A Registered Nurse in this position will have strong communication skills, the competence to work autonomously, professionalism to work within an interdisciplinary team, and the ability to provide care with empathy.

- Graduate of an accredited school of registered nursing.
- Currently licensed as a Registered Nursethroughthe Kentucky Board of Nursingor through a participating Compact State Board of Nursing.
- Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered.
- Demonstrates strong clinical nursing skills and the ability tomaintainprofessionalism during emotionally challenging situations.
- Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
- Adheres to accepted professional, ethical, and legal standards of nursing practice.
- Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
- Maintains current CPR certification.
