Position Summary

The Initiation Clinic Nurse plays a critical role in providing patient-centered care for individuals beginning Medication-Assisted Treatment (MAT) for opioid or substance use disorders. This role conducts clinical assessments, coordinates initiation protocols, educates patients on MAT medications, monitors withdrawal symptoms, and supports safe, evidence-based treatment in collaboration with the Medical Director (MD) and multidisciplinary care team.

This position screens eligible individuals for the Substance Treatment and Recovery (STAR) Program, coordinates closely with the MAT Coordinator for ongoing MAT services, and works with the Outpatient Detox Unit for clients who have higher acuity needs or require stabilization prior to MAT initiation. The Initiation Clinic Nurse also manages daily provider appointment coordination for initiation and will administer all MAT medication injections for Crestview STAR clients.

Applicants must have strong assessment skills, possess the ability to work with behavioral health and substance use populations, and demonstrate motivation to support individuals in recovery.

Minimum Requirements
  • Clear and Active FL Licensed Practice Nurse (LPN) AND be able to obtain a HIV/AIDS 501 Certification as needed. Specific experience in addiction medicine, behavioral health, primary care, or emergency medicine is preferred.

Bilingual/bicultural English and Spanish preferred (not required).

Applicant must be able to pass an Agency for Healthcare Administration (AHCA) Medicaid and Florida Department of Children and Families (DCF) Level II background check, drug screen, and a Tuberculosis (TB) test. Applicant must be at least twenty-one (21) years old and have a valid Florida Driver’s License with no more than three (3) points on their driving record to be insured under BCI’s automobile insurance policy.

Duties / Responsibilities

Some of the job tasks and responsibilities include, but are not limited to:

Patient Assessment & Intake
  • Assist with Bridgeway Orientation for all new clients
  • Perform comprehensive nursing assessments for MAT initiation
  • Obtain medical history, substance use history, vital signs, and withdrawal assessments (COWS, CIWA)
  • Identify contraindications or concerns; report to Medical Provider
MAT Initiation & Monitoring
  • Assist with induction of MAT medications (e.g., buprenorphine/Suboxone®, naltrexone) under provider supervision
  • Monitor/document withdrawal symptoms and response to treatment
  • Communicate timely updates to Medical Provider
Patient Education & Support
  • Provide MAT education including dosing, side effects, safety, harm reduction, and adherence expectations
  • Ensure clear understanding of clinic policies and follow-up appointments
  • Maintain non-judgemental, supportive interactions
Care Coordination
  • Coordinate care with MAT Coordinator, providers, behavioral health, pharmacy, and case management
  • Complete STAR orientation packet and referrals as needed
  • Coordinate with Outpatient Detox Unit for higher acuity clients
