We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality, patient-centered care across various clinical settings, including long-term care, assisted living, and acute care environments. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, medication administration, and comprehensive assessments. Duties
  • Perform thorough patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans.
  • Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care.
  • Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
  • Manage complex care needs for patients with conditions such as dementia, Alzheimer's disease, or those requiring memory or hospice care.
  • Provide wound care, including suturing and dressing changes, while maintaining sterile techniques and infection control standards.
  • Support emergency response efforts by managing airway management, first aid procedures, and acute pain management in urgent situations.
  • Document all patient interactions meticulously using electronic health records (EHR/EHR systems) while ensuring compliance with HIPAA and confidentiality policies.
  • Collaborate with multidisciplinary teams to coordinate discharge planning, case management, and long-term care strategies for optimal patient outcomes.
  • Educate patients and their families on health coaching topics such as medication administration, nutrition, and chronic disease management.
  • Maintain proficiency in clinical procedures like venipuncture, catheterization, tube feeding, and use of medical equipment such as ventilators or cardiac monitors.
Experience
  • Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in healthcare settings.
  • Demonstrated experience working with diverse patient populations across multiple environments including long-term care facilities, hospitals (Level I Trauma Center experience preferred), emergency rooms (ER experience), or outpatient clinics.
  • Proven ability to perform clinical assessments and manage complex cases involving acute pain management or infectious disease care.
  • Familiarity with EMR/EHR systems such as Cerner or Athenahealth for accurate medical documentation.
  • Experience providing specialized care for individuals with developmental disabilities or dementia-related conditions like Alzheimer’s or memory impairments.
  • Knowledge of ICD coding (ICD-9/ICD-10) for accurate documentation of diagnoses.
  • Prior exposure to critical care environments such as ICU or Level II trauma centers is advantageous but not mandatory.
  • Strong communication skills for effective caregiver communication and patient education in fast-paced settings like urgent care or hospital medicine.
