Nurse Aide

• Performs any combination of following duties in care of residents under direction of nursing and medical staff.
• Answers signal lights, bells, or intercom system to determine residents’ needs.
• Bathes, dresses,and undresses residents.
• Serves and collect food trays and feeds residents requiring help after training.
• Transports residents, using wheelchair or wheeled cart, or assists residents to walk.
• Turns andrepositions residents requiring physical assistance, alone or with assistance,to maintain good skin integrity.
• Changes bedlinens, run errands, direct visitors, and answers telephone as needed.
• Food and fluid intake and output, as directed.
• Cleans,sterilizes, stores, prepares, and issues supplies as needed.
• Reports resident status to the nurse on duty and documents in the written medical record as required.
• May be assigned other duties.

Concord Health Systems shall not use any individual working in a facility as a nurse aide if within four (4) months the individual has not completed and passed the nurse aide training and competency testing. (Training provided, if qualified.)
