Horizon House, a non-profit organization, has been widely recognized for its services as a provider of community-based behavioral health, intellectual disabled and homeless services in Eastern Pennsylvania and the state of Delaware. Each year, Horizon House provides a continuum of services at over 100 sites to over 4,500 adults with psychiatric or developmental disabilities, drug and alcohol addictions, and/or homelessness. A pioneer in the field of community-based rehabilitation, Horizon House is widely recognized for its innovative approach to the delivery of services in a community setting.
Currently, we are seeking
Registered Nurse to work in our Developmental Services Division.
This is a split position with zip codes (19468, 19342, 19601)
Purpose:
- Promotes quality of healthcare by developing and interpreting nursing philosophies and standards of care; enforcing adherence to nursing best practices; measuring health outcomes against standards; making adjustments –all in a community-based, licensed, setting.
- Promotes participants’ independence by establishing participant care goals; teaching and counseling participants, Direct Service Professionals (DSPs), family and friends and reinforcing their understanding of disease; medications; medical procedures –to achieve/maintain healthy life styles and self-care skills.
- Maintains and shares knowledge of federal and state healthcare reform.
- Develops, implements and reviews systems to insure quality and cost effectiveness of operations affecting the health, behavioral healthcare and behaviors of all participants with developmental disability and/or Autism.
Supervision Received: - Receives direct supervision from the Regional Director.
Supervision Exercised: - Provide clinical and administrative supervision to Nurses, DSPs and others, as assigned.
Qualifications: - Bachelor's degree in Nursing;
Registered Nurse. Maintain current State of Pennsylvania licensed as RN. - Three to five (3
- 5) years of progressive healthcare management experience. Experience in community nursing and/or developmental disabilities desired. - Prefer experience with electronic health records.
- Excellent interpersonal skills.
- Good organizational skills, good communication skills and detail oriented.
- Proficient use of Microsoft Office Suite for recordkeeping, form development, word processing, and experience with data base design and maintenance.
- Possess valid driver’s license with acceptable driving record.
Scope: - Requires the ability to lift, carry, fold, kneel, reach, stoop, stack, bend, light lifting and climbing stairs.
- May include direct intervention with participants, to include crisis (medical & psychiatric) intervention in emergency situations.
- Frequent talking and listening.
- Requires flexibility in scheduling
- Good manual dexterity and fine motor
- Required to carry a cell phone during regularly scheduled working hours. As member of essential personnel, may be required to report to work during emergencies including inclement weather
