Warren Nursing & Rehab is seeking a compassionate, organized, and dedicated Social Services Coordinator to join our healthcare team.
The Social Services Coordinator plays an important role in supporting residents and their families while helping ensure their psychosocial, emotional, and social needs are addressed throughout their stay.
Position Responsibilities- Provide social services and support to residents and their families.
- Complete social service assessments and maintain accurate, timely documentation.
- Assist residents with adjustment to the facility and changes in their condition or circumstances.
- Serve as an advocate for residents and help address concerns, preferences, and needs.
- Participate in care planning and interdisciplinary team meetings.
- Maintain communication with residents, families, responsible parties, and facility staff.
- Assist residents and families with discharge planning and coordination of community resources.
- Provide information and referrals to appropriate community agencies and services.
- Assist with advance directives, resident rights, and other psychosocial needs.
- Respond to resident and family concerns in a professional and compassionate manner.
- Maintain confidentiality and comply with applicable federal, state, and facility requirements.
- Collaborate with nursing, therapy, activities, dietary, administration, and other departments to promote person-centered care.
- Participate in facility programs, meetings, and quality improvement initiatives as appropriate.
- Bachelor's degree in Social Work, Human Services, Healthcare Administration, Psychology, Sociology, or a related field preferred.
- Previous experience in skilled nursing, long-term care, healthcare, or social services preferred.
- Strong communication, organization, and interpersonal skills.
- Compassionate and professional approach when working with residents and families.
- Ability to handle sensitive and confidential information appropriately.
- Ability to work independently while functioning effectively as part of an interdisciplinary team.
- Knowledge of resident rights, discharge planning, and community resources preferred.
