As a Post‑Acute Care Physician, you’ll manage a panel of patients in skilled nursing and rehabilitation facilities. You’ll play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.
Core Responsibilities- Provide in‑person medical care to patients in assigned post‑acute facilities
- Perform assessments, manage treatment plans, and update documentation in the EMR
- Lead or collaborate with facility teams to ensure quality outcomes and smooth transitions of care
- Participate in Trumen’s value‑based care and transitional care programs
- Develop strong working relationships with facility staff and leadership
- Serve as a liaison between Trumen and facility teams
- Be available in assigned facilities anytime during regular business hours on scheduled days
- MD or DO with a current, unrestricted Texas medical license
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- 1+ year of experience in post‑acute, geriatric, or SNF care preferred
- Comfortable practicing independently in skilled nursing facilities
- Strong clinical judgment, communication skills, and a patient‑centered mindset
- Reliable transportation within the Kerrville area
