Job Description

Under the supervision of the Community Health Improvement Manager or Supervisor (PHN III), you will use the nursing process to assess, plan, implement, coordinate, deliver, and evaluate public health nursing services in the assigned geographic area. You will provide services to individuals, families, groups, and communities, with a focus on wellness promotion and disease prevention, and act as the team leader for DSHS staff in the community.

Responsibilities include managing population‑based services, direct clinic services, responding to state or regional emergencies, and working under general supervision with independent judgment.

Essential Job Functions
  • Adhere to a predictable work schedule while performing duties as assigned.
  • Population‑Based Services (45%):
    Provide essential public health services, conduct community assessments, identify health needs, plan and implement strategies, evaluate outcomes with community stakeholders, develop education programs, establish communication partnerships, and represent DSHS in local coalitions.
  • Direct Care Services (30%):
    Provide specialized public health nursing services in the field office or home, using nursing process to assess and manage clients’ physical, emotional, and psychosocial health, including disease prevention and laboratory testing.
  • Coordination and Team Leadership (15%):
    Evaluate nursing services, orient and supervise staff, manage clinical resources, conduct quality assurance, and prepare program reports.
  • Other Duties (10%):
    Respond to public health emergencies, operate personal or state motor vehicles safely, and comply with the DSHS Immunization Policy.
Registrations, Licensure Requirements or Certifications
  • Registered Nurse license in Texas or a state recognized by the Nurse Licensure Compact.
  • Texas Class C Driver License or equivalent from another state.
  • CPR certification.
  • Valid driver’s license and proof of insurance if operating a vehicle for official duties.
Knowledge, Skills, and Abilities
  • Knowledge of professional nursing theory, nursing process, and applicable regulations.
  • Knowledge of pharmacy and CLIA rules and public health principles, epidemiology, and disease prevention.
  • Knowledge of teaching and learning theories, cultural, social, and economic influences, quality assurance, community resources, assessment tools, and health promotion.
  • Skill in developing and delivering education, assessing complex health problems, providing direct nursing care, basic computer use (data entry, word processing, spreadsheets, email, internet).
