Job Responsibilities and Requirements
Overview & Responsibilities:

• Provide skilled physical therapy services to adult and geriatric patients in a home health setting.

• Perform comprehensive patient evaluations and establish individualized plans of care with measurable short- and long-term goals.

• Deliver evidence-based physical therapy interventions including therapeutic exercise, gait training, neuromuscular re-education, manual therapy, vestibular interventions, pelvic floor training, and patient/family education.

• Complete OASIS assessments, reassessments, recertifications, and discharges in compliance with regulatory and professional standards.

• Manage an average caseload of 4 patient visits per day with an average daily census of 26.

• Treat common diagnoses including COPD, CHF, orthopedic conditions (including joint replacements), neurologic diseases, and cardiopulmonary disorders.

• Monitor and assess vital signs, pain levels, strength, sensory response, cardiopulmonary status, and overall functional mobility.

• Identify environmental barriers in the home and implement safety and injury prevention strategies.

• Collaborate with interdisciplinary team members including social services and interpretation services.

• May float to hospice services within scope if needed.

• Requires reliable personal transportation for community-based home visits.

Required Experience:

• Minimum of 1 year of Physical Therapy experience.

• Minimum of 1 year of Home Health and/or OASIS Assessment (OA) experience required.

• First-time travelers accepted only if OASIS experience is present.

• Epic EMR experience preferred but not required.
Licensure &

Certifications:

• Active Alaska Physical Therapist license required (compact license accepted).

• BLS certification required.
