Shift: Day shift, 4x10-hour shifts (07:00 AM – 05:00 PM); 5x8-hour schedule may also be available; no weekends or on-call required

Job Responsibilities and Requirements
Overview & Responsibilities:

• Provide home health physical therapy services to adult and geriatric patients in a community-based setting.

• Perform comprehensive patient evaluations, develop individualized plans of care, and establish short- and long-term functional goals.

• Deliver skilled physical therapy treatments including therapeutic exercise, gait training, neuromuscular re-education, manual therapy, vestibular interventions, and patient/family education.

• Complete OASIS assessments, reassessments, discharges, and all required documentation in compliance with regulatory and professional standards.

• Manage a caseload averaging 4 visits per day with an average daily census of 26 patients.

• Treat common diagnoses including COPD, CHF, orthopedic conditions (including joint replacements), and neurologic diseases.

• Monitor vital signs, cardiopulmonary status, pain levels, strength, sensory response, and safety risks in the home environment.

• Identify environmental barriers and implement safety and injury prevention strategies.

• Coordinate care with interdisciplinary team members including social services and interpretation services.

• Provide oversight or supervision to therapy assistants, aides, or students as applicable.

• May float to hospice services within scope if needed.

• Requires reliable personal transportation for home visits.

Required Experience:

• Minimum of 1 year of physical therapy experience.

• At least 1 year of Home Health or OASIS Assessment (OA) experience required.

• OASIS experience required for first-time travelers.

• Epic EMR experience preferred (not required).

• First-time travelers accepted with OASIS experience.
Licensure &

Certifications:

• Active Alaska Physical Therapist license required (compact license accepted).

• BLS certification required.
