Join Registered Nurse (RN) Evening at Alternate Solutions Health Network. We CARE for patients like family and offer growth for our team.

Monday, Wednesday, Friday; 2:00 pm – 9:00 pm. No on‑call requirements.

As an RN, you will deliver one‑on‑one patient care, coordinate with interdisciplinary teams, and provide home‑based nursing guided by a physician. Your expertise will help patients spend quality time with loved ones.

Provide skilled nursing care in patients’ homes, work autonomously, manage time efficiently, and perform duties within state scope of practice.

• Plan of Care:
  Observe, monitor conditions, and perform OASIS assessments to develop individualized care plans.
• Minimize Patient Risk:
  Administer medication, prioritize high‑risk patient visits, and reduce rehospitalizations.
• Continuity of Care:
  Manage multidisciplinary care, promote continuity with admissions, transfers, and discharges.
• Family Educator/Advocate:
  Use teach‑back methods to educate patients and families on disease processes.

Complete clinical documentation per agency protocol; collaborate with team and physicians; attend in‑service training and mandatory agency meetings. Follow agency policies, procedures, rules, and regulations.

• Registered Nurse with current state license.
• Minimum one year RN experience in acute care; home‑care experience preferred.
• Valid driver’s license and auto insurance.
• Capable of all physical demands.
