Home Care Worker

* Assist clients with daily activities such as bathing, dressing, meal preparation, and mobility support

* Develop and follow personalized care plans in collaboration with healthcare professionals and family members

* Supervise and support clients to promote their independence while ensuring safety at all times

* Drive clients to appointments, errands, or social activities using your own vehicle or company vehicle

* Maintain accurate records of care provided, including progress notes and any changes in condition

* Communicate effectively with clients, families, and healthcare teams to ensure seamless care delivery

* Monitor client well-being regularly, reporting any concerns or incidents promptly

Qualifications

* Valid driving license with reliable access to a vehicle for client transportation

* Previous experience working in care homes or providing senior care is highly desirable

* Supervising experience or leadership skills to oversee client activities and assist other team members when needed

* Knowledge of creating and implementing care plans tailored to individual needs

* Excellent communication skills for building trusting relationships with clients and their families

* Basic IT skills for maintaining accurate documentation and updating electronic records
