Job Description:- Consults with physicians, patients, hospital staff, family members, or significant others to resolve barriers to treatment progress and ensure continuity of care.
- Maintains records of behavioral health interventions, assessment, diagnostic impression, and treatment for accurate statistics.
- Makes timely referrals to Federal, state, county, and community agencies.
- Develops relationships with agencies to ensure appropriate service delivery to patients.
- Assesses patients for behavioral health needs and provides referrals as needed. Other duties as assigned.
QUALIFICATION REQUIREMENTS:
Master’s (Required) Clinical Social Worker License (LCSW) – EV Accredited Issuing Body, Licensed Marriage and Family Therapist (LMFT) – EV Accredited Issuing Body, Licensed Professional Counselor (LPC) – EV Accredited Issuing Body
Pay Range:$61,560.00 – $
