Treatment Nurse (LVN or RN)

. We are currently seeking a compassionate and detail-oriented Treatment Nurse (LVN or RN) dedicated to providing exceptional wound care and promoting skin integrity for our residents. • Deliver individualized treatment and wound care in alignment with physician orders and established care plans.
• Assess, monitor, and document residents’ skin integrity, collaborating with the nursing and interdisciplinary teams to ensure optimal healing outcomes.
• Partner with physicians and licensed nurses to develop and modify treatment regimens that address both the physical and psychosocial needs of residents.
• Educate residents, families, and staff on skin integrity, wound prevention, and care best practices.
• Maintain compliance with all applicable federal, state, and local regulations, as well as company policies and procedures.
• Support a positive, teamwork-driven environment that upholds Touchstone’s mission and values.

• Current and valid Texas RN or LVN license, or a Compact State RN/LVN/LPN license in good standing.
• Prior experience in skilled nursing, wound care, or long‑term care preferred.
• Strong clinical judgment and attention to detail.
• Excellent communication and interpersonal skills.
