OverviewThe primary function is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence, and coordinate care with the interdisciplinary team, patient/family, and referring agency.
Responsibilities- Under the direction of the RN, participate in planning and implementing specialized procedures and duties that assist in meeting the nursing care needs of the patient by agency policies.
- Assist the RN in carrying out the plan of care.
- Assist the patient in learning appropriate self-care techniques.
- Complete appropriate documentation promptly.
- Provide care utilizing infection control measures that protect both staff and patient (OSHA).
- Provide effective communication to patient/family, team members, and other health care professionals.
- Monitor assigned cases to ensure compliance with requirements of third-party payor.
- Demonstrate commitment, professional growth, and competency.
- Promote agency philosophy and administrative policies to ensure the quality of care.
- Education:
Graduate of an accredited school of vocational nursing. - Learners must hold a current Texas State license as a Licensed Vocational Nurse (LVN) and a Texas Driver’s License.
- Experience:
Two years of experience as an LVN in a clinical care setting; home health experience preferred.
- Nursing skills as defined by generally accepted standards of practice; good interpersonal skills.
- Proof of transportation; reliable transportation with valid and current auto liability insurance.
- Environmental and working conditions awareness; ability to work in patients homes in varying conditions; flexibility to travel locally; some exposure to adverse weather; PRN emergency call.
- Prolonged standing and walking; ability to lift up to 50 lbs and assist with patient movement.
- Ability to work under stress to meet deadlines and patient needs; ability to make quick decisions and resource acquisition; address patient/family psychosocial needs.
- Hand-eye coordination and manual dexterity; current CPR and Hepatitis profile required.
