Licensed Vocational Nurse​/LVN

Tex Med Home Health specializes in offering exceptional care to each client in all stages of life. We provide services to clients in private homes, assisted living communities, hospitals, skilled nursing facilities, and rehabilitation centers. The primary function is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence, and coordinate care with the interdisciplinary team, patient/family, and referring agency.

• Under the direction of the RN, participate in planning and implementing specialized procedures and duties that assist in meeting the nursing care needs of the patient by agency policies.
• Assist the RN in carrying out the plan of care.
• Assist the patient in learning appropriate self-care techniques.
• Complete appropriate documentation promptly.
• Provide care utilizing infection control measures that protect both staff and patient (OSHA).
• Provide effective communication to patient/family, team members, and other health care professionals.
• Monitor assigned cases to ensure compliance with requirements of third-party payor.
• Demonstrate commitment, professional growth, and competency.
• Promote agency philosophy and administrative policies to ensure the quality of care.

• Education:
  
  Graduate of an accredited school of vocational nursing.
• Learners must hold a current Texas State license as a Licensed Vocational Nurse (LVN) and a Texas Driver’s License.
• Experience:
  
  Two years of experience as an LVN in a clinical care setting; home health experience preferred.

• Nursing skills as defined by generally accepted standards of practice; good interpersonal skills.
• Proof of transportation; reliable transportation with valid and current auto liability insurance.
• Environmental and working conditions awareness; ability to work in patients  homes in varying conditions; flexibility to travel locally; some exposure to adverse weather; PRN emergency call.

• Prolonged standing and walking; ability to lift up to 50 lbs and assist with patient movement.
• Ability to work under stress to meet deadlines and patient needs; ability to make quick decisions and resource acquisition; address patient/family psychosocial needs.
• Hand-eye coordination and manual dexterity; current CPR and Hepatitis profile required.
