• Conduct comprehensive patient assessments to determine physical, psychological, and social health needs.
• Develop individualized, evidence-based care plans with specific, measurable goals tailored to patient needs.
• Implement care plans by providing direct patient care, including administering treatments and medications.
• Monitor and evaluate patient progress, document outcomes, and adjust care plans as necessary to achieve desired health outcomes.
• Educate patients and families about self-care techniques, disease management, and the use of medical equipment.
