Generations Family Health Center, Inc. is a regional community health center serving a 37‑town rural region in eastern Connecticut. We provide a full range of primary care services—including medical, dental, behavioral health, care coordination, and community outreach—to all ages from multiple locations (Willimantic, Danielson, Norwich, and Putnam) and through mobile programs and School‑Based Health Centers.

Danielson, CT (multiple sites within the center’s region)

Position Type: Full‑time, 40 hours/week. Primarily day‑time, 8‑hour shifts Monday‑Friday with one Saturday per month (4‑hour shift, stipend). Night or weekend hours may be requested by the Care Team Manager for emergent patient care.

• Providing primary care to adults, pediatrics, and gynecology as assigned, appropriately tailored to patient age.
• Performing office diagnostic and therapeutic procedures, including injections, immunizations, suturing, wound care, and infection management.
• Instructing and counseling patients on therapeutic regimens, growth and development, family planning, emotional health, and health maintenance.
• Developing and implementing patient management plans, recording progress notes, and ensuring continuity of care.
• Conducting comprehensive physical examinations and compiling patient medical data, including health history and exam results.

• M.S. or D.N.P. from an accredited School of Nursing.
• Board certification in the relevant specialty.
• Current licensure in the State of Connecticut or the ability to obtain it.
• Valid Federal and State Controlled Substance License.

Base salary starting at $116,000+ (salary based on experience). Bilingual stipend available for Spanish or Haitian Creole fluency upon passing proficiency assessment.
