Position: Community Nurse (District Nursing)

The Role

The role of community staff nurse will be responsible for the support and management of a caseload of community patients, working in partnership with the District Nurse to assist leading and managing a team of qualified nursing staff and health support workers. You will function as an independent autonomous practitioner by virtue of in‑depth knowledge, expertise, proficiency and experience to provide a high quality nursing service.

You will be responsible and accountable for the ongoing management of patients to ensure the assessment, planning, implementation and evaluation of programmes of care. These will be developed in partnership with patients, family, carers and significant others, and will be evidence based taking into consideration lifestyle, race, gender, cultural background and factors that impact physical, mental or emotional health.

It is essential that you are currently a Registered Nurse with the NMC PIN up to date.

This post requires membership of Disclosure Scotland's PVG.

Duration, Location, and Working Pattern
  • Full‑time and part‑time positions available
  • Shift pattern Monday to Sunday, may include public holidays and out of office hours
  • Based in the North West District Nursing Service
  • Salary is proportional for part‑time hours
Key Responsibilities
  • Assess, plan and implement programmes of care as directed by district nursing sister/charge nurse, developed in partnership with patients, family, carers and significant others, and evidence based taking into consideration lifestyle, race, gender, cultural background and factors that impact physical, mental or emotional health.
  • Assume the responsibility of the caseload holder during short periods of sickness, absence and annual leave, seeking support from senior members of the district nursing team as required.
  • Familiarise with and ensure implementation of local, regional and national policies, procedures and guidelines pertinent to your area.
  • Practice within the legal and ethical framework established by the Nursing & Midwifery Council (NMC) and national legislation to ensure the patients’ interests and wellbeing are met.
  • Maintain accurate up‑to‑date clinical records and care plans in accordance with NMC standards and comply with the Data Protection Act and the Freedom of Information Act.
  • Support patients, families and carers to achieve optimum health and independence as a member of the district nursing team.
  • Plan and prioritise workload, delegate using available resources and make decisions to ensure patients’ needs are met.
Knowledge, training, qualifications and/or experience required to do the job Essential
  • 1st Level Registration or 2nd Level Registration
  • Minimum of 2 years post‑registration experience
  • Evidence of continuing professional development
  • IT skills – word processing, e‑mail, internet
  • Good communication skills (verbal and written)

  • Previous experience in primary care/community setting
