Clinical Pharmacist

Our client is seeking a dedicated and clinically focused Clinical Pharmacist to join their healthcare team in Kimberley . This role involves providing expert pharmaceutical care services to patients, collaborating with physicians and other healthcare professionals to optimize medication therapy and improve patient outcomes. You will play a vital role in medication safety, drug information services, and potentially specialized areas like anticoagulation or infectious disease management.

Key Responsibilities   Review and interpret physician orders, ensuring appropriate drug selection, dosage, and route of administration.
Provide comprehensive medication management services, including medication reconciliation, patient counseling, and adherence monitoring.
Collaborate with the interdisciplinary healthcare team to optimize drug therapy and address drug-related problems.
Serve as a primary resource for drug information to physicians, nurses, and other healthcare providers.
Participate in patient care rounds and contribute to the development and implementation of evidence-based treatment protocols.
Monitor drug therapy for adverse effects, drug interactions, and therapeutic efficacy, intervening as necessary.
Requirements   Bachelor of Pharmacy degree or Doctor of Pharmacy (PharmD) degree.
Registered Pharmacist license in good standing.
Minimum of 3 years of experience practicing as a Clinical Pharmacist , preferably in a hospital or ambulatory care setting.
Strong knowledge of pharmacotherapy, drug interactions, and pharmaceutical care principles.
Excellent communication, interpersonal, and problem-solving skills.

Experience with electronic health record (EHR) systems and pharmacy information systems.
