About the Role  Our client is seeking an experienced and organized Case Manager to join their Social Services team in Kimberley . This role focuses on assessing client needs, developing individualized support plans, and coordinating services to help clients achieve their personal and social goals. You will work closely with individuals and families facing various challenges, connecting them with essential resources and advocating on their behalf.

This hybrid position requires both in‑person client engagement and remote administrative duties, offering a balanced approach to case management.

Key Responsibilities   Conduct comprehensive client assessments to identify needs, strengths, and barriers.
Develop, implement, and monitor individualized case plans in collaboration with clients.
Coordinate access to a range of social services, including housing, employment, healthcare, and financial assistance.
Provide ongoing support, counseling, and advocacy for clients throughout their case journey.
Maintain accurate, detailed, and confidential client records and progress notes.
Liaise with community partners, government agencies, and other stakeholders to ensure integrated service delivery.
Conduct regular follow‑ups to evaluate client progress and adjust support plans as needed.
Requirements   Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
Minimum of 3 years of experience in case management or social services.
Solid understanding of social welfare systems, community resources, and relevant legislation.
Excellent assessment, planning, communication, and interpersonal skills.
Proficiency in case management software and virtual communication tools.
Ability to manage a caseload effectively, balancing in‑person and remote tasks.
