Key Responsibilities:
• Assess individual needs and create personalized support plans
• Coordinate therapy, assistive technology, and personal care services
• Liaise with healthcare professionals and community organizations
• Monitor service effectiveness and adjust plans as needed
• Provide clients and families with information on resources
Requirements:
• Bachelor’s in Social Work or related field
• 3+ years in disability services or case management
• Knowledge of advocacy, legislation, and support services
• Excellent interpersonal and problem-solving skills
• Proficient in digital tools and case management software
