About the Role

Our client is seeking a highly organized and dedicated Case Manager to join their team in Kimberley . This hybrid role involves assessing client needs, developing personalized support plans, and coordinating services to help individuals and families achieve their goals. You will work within the community and utilize remote communication tools to manage client cases effectively, ensuring they receive the necessary assistance for social, health, or economic well-being.

• Conduct thorough assessments of client needs, strengths, and challenges.
• Develop, implement, and monitor individualized case management plans.

• Liaise with various agencies, service providers, and stakeholders to coordinate care.
• Maintain accurate and up-to-date case files and documentation.

• A relevant qualification in Social Work, Social Sciences, or a related field.
• Previous experience in case management, social support, or community services.
• Strong understanding of available social services and resources in the area.
• Excellent organizational, communication, and problem-solving skills.
• Ability to manage a caseload and prioritize tasks effectively.
• Proficiency in using case management software and virtual communication tools.
