Physical Therapist Assistant, Home Health

Join a caring community dedicated to restoring strength, mobility, and independence for patients in the comfort of their homes. You will provide personalized care, support rehabilitation, and improve quality of life.

Responsibilities
  • Administer the physical therapy treatment plan as developed by the Physical Therapist.
  • Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
  • Accurately document daily, weekly, or monthly records of clients and submit clinical paperwork to the office on time each week.
  • Attend educational meetings and coordinate and contribute to department in‑services.
  • Notify the physical therapist or Executive Director of any equipment failures, lack of supplies, staff or client changes, and other departmental issues.
  • Schedule clients and perform varied office tasks.
  • Apply concepts of infection control and universal precautions in performing patient care activities.
  • Accept clinical assignments consistent with education and competence to care for patients.
  • Promote compliance with payors through education, coaching, and other assistance as necessary.
Qualifications
  • Associate degree in Physical Therapy from an American Physical Therapy Association.
  • Current state license to practice Physical Therapy.
  • Valid driver’s license, auto insurance and reliable transportation.
  • Proof of current CPR certification.
  • Minimum two years experience as a Physical Therapy Assistant.
  • Comprehensive knowledge of geriatric population changes.
  • Self‑directed, enthusiastic, and accepts constructive feedback.
  • Good verbal and written communication and organization skills.
  • Professional image/attitude for patients, families, and coworkers.
  • Meets mandatory continuing education requirements of Center Well Home Health and licensing board.
  • Flexible schedule and local travel ability.
  • Maintains confidentiality.
  • Fluent English.
  • Regular attendance.
Pay and Benefits

Base pay: $58,400 – $80,000 per year. Pay per visit/unit: $37.00 – $52.00. Benefits include medical, dental, vision, 401(k), paid time off, disability, and life insurance.
