Job Summary

Position Type:
Non‑exempt, full‑time or part‑time, hourly role.
Scope of Role:
Provide direct support within a single program or unit, typically supporting 4‑6 individuals in residential settings or up to 10 individuals in periodic programs.

Key Responsibilities
  • Deliver service plans including socialization, health maintenance, medication administration, skill/behavioral development.
  • Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
Essential Job Functions
  • Direct Support:
    Assist socialization and behavioral development, personal care, housekeeping, recreation, transportation, shopping, financial management, citizenship, and other activities of daily living as outlined in individual service or program plans.
  • Records:
    Accurately document progress and activity; review records and logs to stay abreast of changes; maintain confidentiality.
  • Management of Individuals’ Assets:
    Make cost‑effective purchases within the individual’s budget and according to their preferences.
  • Relationships:
    Maintain healthy professional relationships with individuals, families, guardians and case managers; greet visitors; implement Sevita Customer Service Standards.
  • Reporting:
    Report incidents of alleged abuse or neglect, medical or behavioral incidents, and other events in accordance with Sevita policy and external requirements.
  • Rights:
    Maintain confidentiality, respect rights of persons served, practice universal precautions, and assist individuals in exercising their rights.
Health Care
  • Appointments:
    Accompany individuals to medical appointments as needed and relay orders and information to providers.
  • Medications:
    Accurately administer or support self‑administration; document delivery; report errors; maintain security of controlled medications.
  • General Health Care:
    Monitor health, document concerns, communicate with nurses or supervisors, call 911 in emergencies, and follow health care directives.
  • Medical Supplies and Equipment:
    Use equipment correctly and report malfunctions.
  • Meal Preparation:
    Prepare or support individuals in food preparation according to menus, dietary restrictions, and dining plans.
Employment Responsibilities
  • Training:
    Attend orientation and ongoing training; participate in monthly staff meetings.
  • Employee Scheduling:
    Work scheduled hours, arrive on time, stay until replacement arrives, notify supervisor of absences promptly, and fill shifts when required without causing unapproved overtime.
  • Teamwork:
    Work collaboratively with others, value contributions, communicate openly, listen, and show respect.
  • Workplace Safety:
    Comply with safety policies, report hazards, and participate in safety training or activities.
Maintenance
  • Vehicles:
    Transport individuals safely, use safety equipment, and report accidents or safety concerns immediately.
  • Housekeeping:
    Perform daily and seasonal housekeeping; maintain a safe environment and report hazards.
  • Maintenance and Repair:
    Monitor safety, make minor repairs, perform yard maintenance, and report equipment or facilities needing repair.
  • Safety:
    Check water temperature during bathing; participate in safety drills and protect served persons during emergencies.
Other Duties
  • Perform other duties and activities as required.
  • MPA = Mobile Punch Authorized for timekeeping.

  • None
Minimum Knowledge and Skills
  • High school diploma or equivalent preferred.
  • Six months of human services experience preferred.
  • Must be 18 years of age.
  • Working knowledge of computers.
Certificates, Licenses, and Registrations
  • Valid driver’s license in good standing.
  • Car registration and vehicle insurance if transportation is provided.
  • All state‑required training(s) and certification(s) completed in mandated time frames.
