Job DescriptionPosition Purpose: Registered Nurse (RN) coordinates the total plan of care for the patient in the home care setting assessing, planning, implementing and evaluating.

The Home Health RN coordinates the implementation of a nursing plan of care for each assigned patient. Works under the direction of the physician and the Arizona State Nurse Practice Act to provide nursing care to patients in compliance with nursing standards and regulations. Utilizes the nursing process to make continuing assessments of the patient’s total health needs; implements nursing interventions based on assessment data;
develops and implements the nursing plan of care and evaluates the patient’s response. Collaborates with healthcare providers, nursing staff, other patient caregivers, case managers, and health information team members to improve the quality and completeness of documentation of care. Adheres to behavioral expectations and policies; maintains patient and staff confidentiality; supports a healthy work environment through skilled communication, true collaboration, effective decision making, and meaningful recognition.
- Graduate of a nursing school program
- AZ RN License, or compact state RN License
- Level One Fingerprint Clearance Card, or application, required within 20 days of hire
- AZ Driver's License
- BLS Certification from American Heart Association or American Red Cross
- Must be at least 21 years of age
- 1-2 years acute care nursing or home health experience
Requires travel throughout local service area and interactions with customers in their home setting. Duties have routine potential for exposure to blood, body fluids or tissues; ability to move patients >300lbs; sit, stand, walk, and bend during the work shift. Ability to address multiple priorities simultaneously, possess good oral and written communication skills, knowledgeable with business office equipment.
