Family Nurse Practitioner / Adult-Gerontology Nurse Practitioner Overview & Highlights

About the Opportunity

This is more than a traditional Nurse Practitioner position. It is an opportunity to become an early clinical leader in a growing geriatric healthcare organization addressing a significant shortage of care throughout Lake Havasu and the surrounding region.

Many older adults in this community have difficulty accessing consistent primary care. Some must travel hours for appointments, while others lack a dependable provider who understands their medical history, living environment, family dynamics, and long-term goals.

You will help change that.

As an FNP or AGNP, you will provide comprehensive primary care to older adults living in assisted living, memory care, and independent living communities. You will build lasting relationships with patients, families, facility leaders, caregivers, and community partners while helping establish a higher standard of accessible, responsive geriatric care.

The organization’s care philosophy is informed by person-centered and Montessori-based principles. Patients are treated as individuals with purpose, preferences, abilities, and meaningful lives, not simply as diagnoses or a collection of medical conditions.

Training and Ramp-Up

You will receive a level of hands-on support that is uncommon in highly autonomous provider roles.

Your initial training is expected to include:

  • Direct support from the organization’s owners and clinical leadership
  • Clinical guidance and physician collaboration
  • Training with an experienced Nurse Practitioner consultant
  • Dedicated Nurse Practitioner coaching during the first several months
  • Education regarding geriatric care delivery in senior living communities
  • Documentation, billing, coding, credentialing, and compliance training
  • Coaching on communicating with patients, families, facility leaders, and caregivers
  • Support developing an efficient and sustainable daily workflow
  • Ongoing feedback focused on identifying your strengths and accelerating your professional growth

The organization’s leadership team will be highly involved during the launch. You will not be expected to build the practice alone or figure everything out without support.

What the First Year May Look Like

First 30 Days

The initial month will focus heavily on training, onboarding, credentialing, documentation, billing processes, clinical expectations, community introductions, and preparation to begin building the patient panel.

Months Two Through Six

You will progressively increase patient visits and community rounding across assisted living, memory care, and independent living communities.

A typical day may eventually include approximately 7 to 12 patient interactions, depending on patient complexity, travel, telehealth visits, care coordination, and administrative responsibilities.

During this launch period, patient and community needs may require availability across six days and participation in an on-call schedule. Responsiveness to urgent patient, family, and facility concerns will be essential.

After the Initial Ramp-Up

Leadership will evaluate patient volume, workload, scheduling, and the provider’s professional goals. The intention is to create a sustainable practice model that offers meaningful autonomy, flexibility, and room for continued growth.
