This is an exciting opportunity for existing clinicians or newly qualified mental health nurses/social workers/occupational therapists to join the West Youth Community Mental Health team based at Thurlow House in Kings Lynn as a Band 5 Mental Health Practitioner. This is a full time (37.5 Hours) post, working with service users ages 14-25 years of age.
You will be working in a highly skilled multi-disciplinary team with a strong team ethos of supporting each other. You will be supported to develop a strong understanding of your role as a Care Coordinator/Assessor becoming skilled at delivering evidence-based treatment interventions for our service users. Working in partnership with other agencies will be a big part of the job so good communications skills will be vital.
What you can expect is a thorough induction and training / preceptorship period, and opportunities for ongoing development and well as regular and supportive managerial and clinical supervision.

Day to day the role will involve core duties some of which are listed below:
- Managing a caseload via face to face, online and telephone contacts
- Waiting List monitoring via telephone calls
- Undertaking initial assessments on behalf of the service
- Undertaking face to face visits in the community setting including home visits
- Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders
- Attending regular MDT meetings and liaising with colleagues from other services (both internally and externally such as Social Care, Crisis Resolution and Home Treatment Team, U18 Crisis Team CAIST, Adult Community Mental Health Teams, MHT, Acute Hospitals A&E, and third sector organisations)
- Participating in the duty worker rota (Monday to Friday 09:00am to 17:00pm) may be part of this role moving forward
Valid UK Driving licence is required for this post.
