Rapid Assessment & Frailty Team Paramedic/Therapist/Nurse

The integrated Rehabilitation Service, is looking for an enthusiastic staff member who wishes to develop their clinical skills in a dynamic and supportive environment.

The successful candidate will be based within the Rapid Assessment and Frailty Team and the role will involve delivering high quality assessment, treatment and advice for patients within our emergency areas. You will concentrate on admission avoidance and facilitating early discharge. You will manage patients with diverse and often complex needs whilst working as part of a supportive team.

This role will include weekend working as part of an agreed rota.

Main duties of the job

As a senior member of the multidisciplinary team, you will play a key role in ensuring the smooth and effective delivery of services in conjunction with your colleagues within the team. You will manage your own caseload independently, ensuring safe and seamless transfer of patients through their journey in the Queen Elizabeth Hospital, enabling safe and timely discharges.

You will have the opportunity to develop your management and teaching skills by leading on in-service training, supervision of other staff members and mentoring of students.

Strong communication skills are essential, as you will liaise closely with patients, carers and colleagues across the multidisciplinary team, acting as an advocate to safeguard patient well being and promote a positive experience.

Person Specification Qualifications/training and professional development
  • Degree in Nursing, Physiotherapy, Occupational Therapy, Paramedic Practice or Paramedic Science.
  • Registered General Nursing Registration, Health & Care Professions Council registration.
  • Demonstration of a variety of post‑graduate courses to increase levels of skill and knowledge.
  • Practice Educator course and ability to supervise under‑graduate students from your profession.
Experience
  • Covered a variety of specialities as a post‑graduate Nurse, Therapist or Paramedic.
  • Experience of multi‑professional working.
  • Broad range of NHS experience.
  • Experience of working in the emergency care environment.
  • Experience of community care.
Skills, abilities and knowledge
  • Ability to make sound clinical judgements and decisions in response to complex or crisis situations.
  • Ability to prioritise one's own workload and utilise the skill of delegation as appropriate.
  • Knowledge of the Comprehensive Geriatric Assessment process.
  • Experience of staff supervision and appraisal.
  • Positive approach to data collection and clinical audit.
