Nurse Practitioner
Bayshore Health Care is a leading provider of home and community health care services in Canada. We are currently hiring a full‑time Nurse Practitioner to join our Transitional Care Unit in Amherstview, Ontario, supporting seniors transitioning from hospital to community.
Duties and Responsibilities- Support the needs of patient populations with innovative approaches to care delivery.
- Provide comprehensive primary care as part of an interdisciplinary team focused on creating person‑centered plans of care.
- Review and update primary care activities in line with organizational goals and projected outcomes.
- Promote evidence‑based practice through the use of the latest research‑based guidelines.
- Ensure the provision of safe, evidence‑based, cost‑effective, patient and family‑centered care.
- Identify and implement appropriate clinical documentation and record‑keeping practices.
- Keep up to date with health‑related policy and implement changes to positively impact patient care.
- Participate in ongoing quality monitoring and improvement.
- Create and maintain an exceptional quality program.
- Create an environment that builds on the principles of an integrated team approach.
- Completion of a recognized Primary Care or Adult Nurse Practitioner program with a minimum of 3 years of geriatric experience.
- Ability to prescribe controlled substances as authorized by the College of Nurses of Ontario.
- Master of Nursing degree.
- Current Extended Class certification with the College of Nurses of Ontario.
- Sound working knowledge of hospital nursing functions and current needs in nursing, health care and geriatric care.
- Previous experience in clinical work with geriatric clients, including diagnosis and staging of dementia.
- Demonstrated ability to exercise independent judgment and assess situations and problems efficiently and effectively.
- Effective verbal and written communication skills.
- Excellent interpersonal skills, experience with written documentation, care plans and dictation.
- Strong interpersonal, organizational, collaborative, and problem‑solving skills.
- Ability to work well within an inter‑professional team and foster effective working relationships.
- Ability to exercise initiative and observe strict confidentiality.
- Expert wound care and IV skills.
