Position Overview Registered Practical Nurse (RPN) – Community Nursing. In this role you will conduct scheduled visits within the community to care for patients of all ages in various settings. You will work with patients of differing complexity of care needs and help them remain comfortable in their homes for as long as possible.
Responsibilities Participate in scheduled community visits to address patient needs within your scope of practice.
Provide ethical and safe nursing care.
Use appropriate assessment tools and techniques to evaluate patient needs.
Execute and continuously monitor care plans, adjusting as required.
Engage patients collaboratively by building rapport and trust.
Maintain and update professional patient records related to encounters and plans.
Qualifications Registered with the CNO as a Registered Practical Nurse in good standing.
Valid driver’s license and access to a vehicle.
Experience delivering community care (asset).
Reliability and flexibility.
Willingness to travel to and from patient residences within a designated area.
Experience working with vulnerable populations.
Passion for making a difference in the community.
Required on‑call coverage, including after‑hours to ensure patient safety and meet care needs.
Bilingualism (English/French) preferred.

Compensation Full‑time: $26.42–$30.50 per hour, based on skills, experience, education, and alignment with responsibilities.
