Position: Nurse Practitioner for Ambulatory Programs
Be a vital part of outpatient care as a Nurse Practitioner in Ambulatory Programs. Manage patient wellness across various clinics while showcasing your expertise in chronic disease management.
This temporary full-time position is ideal for an experienced nurse with a master’s degree in nursing and current registration as a Registered Nurse in the Extended Class. You will work alongside a dedicated health care team supporting numerous ambulatory programs and ensuring effective care coordination for a diverse patient population.

Key Responsibilities:

• Provide patient-centered care in outpatient settings

• Communicate effectively with interdisciplinary teams

• Manage transitions of care smoothly

• Support quality improvement efforts in the program

• Address the needs of patients with chronic conditions
Requirements:

• Master’s degree in nursing required

• At least 3 years of nursing experience in relevant fields

• Active registration with the College of Nurses of Ontario

• Strong critical thinking and teamwork skills

• Must be fully vaccinated against COVID-19
