Job Description ### About the Role We are a healthcare recruitment team hiring on behalf of a client for two (2) full-time Nurse Practitioner opportunities supporting community-based primary care across Brockville & Mallorytown. In this role, you’ll deliver comprehensive, patient-centered care and collaborate with an interdisciplinary team across prevention, treatment, chronic disease management, and health promotion.
Region / Work Area :

Compensation & Schedule
* Pay: $68.55/hour
* Hours:
37.5 hours/week (weekdays + some evenings; flexible work week)
Key Responsibilities
* Provide comprehensive primary care: assessment, diagnosis, treatment, follow-up, and referral
* Perform advanced health assessments; order/interpret diagnostics; prescribe and manage medications within scope
* Manage acute and chronic conditions; support complex patients with ongoing care plans
* Deliver health promotion, disease prevention, and patient education/counselling
* Collaborate with physicians and allied health professionals in an interdisciplinary setting
* Maintain timely, accurate, and complete clinical documentation (EMR/clinical records)
* Support care coordination, continuity of care, and patient navigation to community resources
* Contribute to quality improvement and evidence-informed practice
Qualifications & Experience
* Nurse Practitioner (Primary Care & Adult) with current registration in good standing with the College of Nurses of Ontario (CNO)
* Strong clinical judgment, critical thinking, and decision-making ability
* Ability to work independently while thriving in a collaborative, interdisciplinary environment
* Community-based primary care experience and/or experience supporting individuals with complex needs (asset)### Skills
* Excellent verbal/written communication and strong interpersonal skills
* Highly organized; accurate; strong attention to detail
* Strong computer skills; confident with clinical documentation standards
* Demonstrated commitment to respectful, inclusive care across diverse backgrounds### Requirements
* Satisfactory Vulnerable Sector Check
* Valid driver’s license and $2M vehicle liability insurance
