DESCRIPTION: The Nurse Practitioner (NP) is a Registered Nurse in the Extended Class with advanced education and clinical training who provides comprehensive, patient-centered care across the continuum of health services. As the Most Responsible Provider (MRP), the NP-MRP carries full responsibility for autonomously assessing, diagnosing, ordering and interpreting diagnostic tests, initiating treatment plans, prescribing medications, and discharging patients within their legislated scope of practice.
The NP-MRP holds full responsibility for coordinating and managing patients’ care, ensuring timely transitions, and facilitating appropriate follow-up.
Functioning within a framework of advanced nursing practice, the NP-MRP may collaborate with members of the interprofessional team to optimize patient outcomes through evidence-informed clinical decision-making, proactive discharge planning, and care coordination. The NP-MRP consults with physicians as appropriate (e.g. for specialist care or transfer of care if patient care needs fall outside the NP-MRP scope). The NP-MRP’s practice also includes significant contributions to education, leadership, and research/quality improvement initiatives.
The role requires consistent application of professional standards, best practice guidelines, and regulatory requirements, while actively contributing to strategic priorities related to patient flow, service excellence, and interprofessional collaboration.
In addition, the NP-MRP acts as a clinical leader and mentor, supports continuous learning for staff and students, and engages in system-level initiatives that enhance the quality of care and nursing practice. The NP-MRP is expected to model professional conduct, champion innovation, and advocate for patients and families while supporting organizational goals and maintaining compliance with relevant legislation and hospital policies.
Within this role the employee is accountable for contributing to the delivery of the Kingston Health Sciences Centre strategy. As an employee, one must demonstrate an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care in all we do.
PRIMARY RESPONSIBILITIES & DUTIES:Clinical Domain
The NP-MRP works autonomously to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within the legislated scope of practice.
- Assess, analyze and synthesize a wide range of information using academic evidence and advanced clinical knowledge to develop a patient plan of care.
- Holds overall accountability for a patient’s care and participates in timely patient flow planning, preventing delays by collaborating other members of the interprofessional team and leadership as appropriate.
- Manages patients autonomously within their scope of practice and is responsible for arranging transfer of care to a physician when the patient’s care exceeds the clinical scope of the NP-MRP.
- Serves as the primary point of contact for the patient, their family, and the care team. The NP-MRP ensures continuity of care is maintained during patient transitions in care, such as discharge, and will ensure appropriate follow-up is coordinated
The NP-MRP will identify risk related to clinical practice which is managed through observation, assessment, and discussion with the involved interprofessional team members.
- Improve clinical practice through the development, implementation and evaluation of practice guidelines, standards, policies and procedures that promote excellence in nursing practice in collaboration with key stakeholders.
