Elevate your nursing career with Providence Transitional Care Centre as a Registered Practical Nurse focused on patient-centered care. Work part time to make a meaningful impact on the lives of patients in a supportive environment.
This role requires you to operate within a fully integrated team, providing care based on established nursing standards. You'll be responsible for assessing patients' health statuses and delivering high-quality care. Additionally, you will prioritize care needs and advocate for patient and caregiver involvement in treatment decisions while monitoring and adjusting care plans for optimal outcomes.
Key Responsibilities:
• Demonstrate competencies for safe patient care delivery
• Evaluate health status for patients with well-defined care needs
• Establish and prioritize preventive and rehabilitative care
• Act as an advocate for patient and caregiver involvement
• Modify care plans based on patient evaluations and feedback
Requirements:
• Unrestricted license to practice in Ontario
• Valid CPR certification
• Nursing college certificate or diploma preferred
• Medication Certificate if graduated before 1985
• Effective communication and professional judgment skills
