This full-time role involves direct engagement with patients and their families to facilitate recovery. You will utilize your clinical knowledge to coordinate services and ensure that individual needs are met effectively. With an emphasis on building therapeutic relationships, you will play a pivotal role in guiding clients towards achieving their wellness goals.
Key Responsibilities:
• Establish therapeutic relationships with patients and families
• Coordinate and adjust delivery of treatment and rehabilitation
• Evaluate and document medication effectiveness and complications
• Provide skill training and assist with daily living tasks
• Support clients in developing community connections
Requirements:
• Registered Nursing diploma; degree highly regarded
• Valid registration with the College of Nurses of Ontario
• Minimum three years providing recovery-focused nursing
• Preferred experience in forensic nursing
• At least one year working with serious mental health conditions
Drive recovery and independence in your nursing practice as an RN Case Manager.
