Leeds & Grenville Community Family Health Team is looking for a passionate Registered Nurse to engage in collaborative care. With at least five years of experience, you’ll perform key nursing tasks including history taking and immunizations. Proficiency in electronic medical records is essential, as are advanced assessment and communication skills for effective patient interaction.

Key Responsibilities:

• Conduct thorough health history taking for patients

• Perform nursing procedures meeting professional standards

• Administer immunizations and provide health education

• Document patient information in electronic medical records

• Collaborate with an interdisciplinary care team

Requirements:

• Active RN license with the College of Nurses of Ontario

• Minimum five years’ nursing experience

• Strong assessment and communication skills

• Flexible availability for evenings and weekends

• Valid Driver’s License and vehicle access
