Key Responsibilities:
• Participate in community visits to assess patient needs
• Provide ethical and safe nursing care to individuals
• Use assessment tools to evaluate patient conditions
• Execute and monitor tailored care plans effectively
• Engage with patients, building rapport and trust
Requirements:
• CNO registration as a Registered Practical Nurse
• Valid driver’s license and access to a vehicle
• Community care experience is an asset
• Flexibility to meet varying patient needs
• On-call coverage commitment
