As part of the Leeds & Grenville Community Family Health Team, you will draw upon your extensive nursing experience to work across various sites, including Brockville and Gananoque. Ideal candidates should possess strong assessment and communication skills to support patient care through immunizations and health education. This role requires familiarity with electronic medical record systems for efficient information management.

Key Responsibilities:

• Take detailed patient histories during consultations

• Perform nursing procedures aligned with Ontario standards

• Administer immunizations and offer health counseling

• Keep accurate patient records using electronic systems

• Work cohesively within an interdisciplinary team

Requirements:

• Valid RN license in good standing in Ontario

• At least five years of nursing experience

• Excellent communication and assessment abilities

• Availability for evening and weekend shifts

• Access to a vehicle with a valid Driver’s License
