Position: Ambulatory Care Nurse Practitioner Role

This role reports directly to the Manager of the Ambulatory Program, focusing on key areas such as the Nurse Practitioner clinic and Internal Medicine clinics. You will utilize your master's degree in nursing along with your skills in outpatient services and chronic disease management to enhance patient outcomes. A minimum of three years of nursing experience is required for this collaborative and dynamic environment.

Key Responsibilities:

• Deliver comprehensive care in ambulatory settings

• Collaborate with health care providers and interdisciplinary teams

• Manage and direct care for designated patient populations

• Support and lead quality improvements in the program

• Communicate effectively with primary care providers

Requirements:

• Current RN registration with extended class specialization

• Master of Science in Nursing or related field

• Minimum three years of nursing experience

• Strong critical thinking and problem-solving skills

• Must be double vaccinated against COVID-19
