PRIMARY FUNCTION

The Nurse Practitioner (NP) is a Registered Nurse (RN) in the Extended Class with additional educational preparation and experience, who possesses and demonstrates the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform specific procedures within their legislated scope of practice.

The NP must know and practice in accordance with the standards relevant to the practice area as well as the legislation and regulations applicable to an NP practice:

• Regulated Health Professions Act, 1991;
• Nursing Act, 1991;
• Laboratory and Specimen Collection Centre Licensing Act;
• Healing Arts Radiation Protection Act, 1990;
• Public Hospitals Act, 1990;
• Health Protection and Promotion Act, 1990.

Reporting to the Program Operational Director, the NP works in an extended nursing role demonstrating a high level of autonomy and expert skill in order to formulate clinical decisions, appropriately manage acute illness and promote wellness within their area of specialty.

The NP explicitly consults the interprofessional team where care is required outside of the NP scope of practice and/or not covered by a Medical Directive.

Within this role, the NP is accountable for contributing to the delivery of the Kingston General Hospital strategy and must demonstrate an awareness of and be responsible for actively promoting and supporting patient-and-family-centered care and engagement.

PRIMARY RESPONSIBILITIES & DUTIES:

Clinical Domain

The NP works autonomously to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within the legislated scope of practice.

• Assess, analyze and synthesize a wide range of information using academic evidence and advanced clinical knowledge to develop a patient plan of care.
• Holds overall accountability for a patient’s care and participates in timely patient flow planning, preventing delays by collaborating other members of the interprofessional team and leadership as appropriate.
• Manages patients autonomously within their scope of practice and is responsible for arranging transfer of care to a physician when the patient’s care exceeds the clinical scope of the NP.
• Serves as the primary point of contact for the patient, their family, and the care team. The NP ensures continuity of care is maintained during patient transitions in care, such as discharge, and will ensure appropriate follow-up is coordinated.

The NP will identify risk related to clinical practice which is managed through observation, assessment, and discussion with the involved interprofessional team members.

• Improve clinical practice through the development, implementation and evaluation of practice guidelines, standards, policies and procedures that promote excellence in nursing practice in collaboration with key stakeholders.
• Participates in clinical and organizational committees
• Provide leadership by supporting and fostering evidence-based practice and continuous quality improvement processes within the organization.
• Acts as a resource to individuals and groups within the Program and throughout the organization by promoting and ensuring excellence in nursing practice.
• Provides leadership and consultancy in professional practice to other nursing colleagues within and outside the organization locally and regionally, and to other members of the interprofessional care team.
• Advocates for individuals, families, groups and communities in relation to treatment, the health-care system and policy decision that affect health and quality of life.
• Identifies learning needs of nurses and other members of the interprofessional team and will address those needs within resources.
