As a Registered Practical Nurse with SE Health, you will play a vital role in the Acute Care Transitions Program. Your expertise in nursing will allow you to deliver comprehensive care and conduct joint care planning with interdisciplinary teams in a client-centered manner. Key Responsibilities:
• Provide comprehensive care within an interdisciplinary team
• Serve as primary contact for client’s care plans
• Conduct client assessments and develop tailored care plans
• Facilitate interdisciplinary communication and meetings
• Organize family and caregiver meetings during transitions
Requirements:
• Registered with the College of Nurses of Ontario
• Two years of nursing experience required
• Experience in home/community care or acute care
• Excellent assessment and organizational skills
• Valid driver's license and ability to travel
