Become a Geriatric Nurse Coordinator at Providence Care Hospital, specializing in ambulatory care for older adults. Leverage your nursing skills to meet complex health challenges.
In this full-time role, you will deliver advanced nursing care to elderly patients with multiple health issues as part of the ambulatory care program. Your responsibilities include clinical triage, comprehensive assessments, and active collaboration with healthcare providers to ensure effective patient management. This role plays a vital part in improving patient access and care delivery.
Key Responsibilities:
• Evaluate incoming referrals to ensure alignment with care pathways
• Perform detailed nursing assessments covering all care aspects
• Manage clinic schedules and anticipatory patient care actions
• Deliver individualized patient education using health literacy
• Act as a key resource within the outpatient geriatrics team
Requirements:
• Bachelor’s Degree in Nursing
• Current, unrestricted license with College of Nurses of Ontario
• Three years’ experience in geriatrics or senior health
• Experience coordinating complex care in healthcare settings
• Commitment to complete required geriatric training
