About the Transitional Care Unit (TCU)

The Transitional Care Unit (TCU), located at the Alternative Healthcare Facility in Amherstview, is a 120-bed program supporting patients who require ongoing care while awaiting their next stage of care. Patients may be:

  • Working toward increased independence through rehabilitation
  • Receiving palliative and comfort-focused care

Our Nurse Practitioner-led model promotes a collaborative approach that allows team members to build meaningful relationships with patients and contribute to their ongoing care journey.

What You'll Do

As a Registered Practical Nurse, you will:

  • Assess, plan, implement, and evaluate patient care within the RPN scope of practice
  • Deliver evidence-informed nursing care focused on patient goals and outcomes
  • Monitor patient progress, identify changes in condition, and collaborate with the healthcare team as needed
  • Develop and coordinate individualized care plans
  • Support patients and families through rehabilitation, transitional care, and palliative care journeys
  • Delegate, teach, and supervise unregulated care providers
  • Participate in discharge planning and care coordination
  • Collaborate with physicians, allied health professionals, patients, and families to ensure seamless patient care
  • Contribute to quality improvement initiatives that enhance patient experiences and outcomes
  • Mentor students and team members while fostering a positive practice environment

Required Qualifications
  • Diploma from a Registered Practical Nurse program
  • Current registration in good standing with the College of Nurses of Ontario (or eligible for registration)
  • Demonstrated knowledge of CNO standards and RPN scope of practice
  • Excellent assessment, communication, and organizational skills
  • Strong problem-solving and decision-making abilities
  • Basic computer proficiency
  • Ability to work effectively in a fast-paced healthcare environment
  • Proven ability to attend work regularly
  • Satisfactory Criminal Record Check and Vulnerable Sector Screening
